Thanks for all the suggestions. I will be incorporating them into my workouts/rest from now on.
Looking back I just overdid it. Not enough breaks, too much accessories thrown in. I like to sneak in a few things from time to time.
Re-reading some of the literature (TBIII & AA) I need to just back off some of the extra stuff and take some off weeks. I rested all this week, only doing light shoulder maintenance lifts (TYWL). Going to run the same numbers for again 3 weeks. Then I will see about forcing progression.
I liked the suggestion I read saying if the 95% double week is pretty easy, then that is a good signal to force progression on that lift. If it seems pretty difficult, run the same numbers again.
